This California Notice at Collection is provided by euVita Health, Inc. (“euVita,” “we,” “us,” or “our”) for California residents pursuant to the California Consumer Privacy Act, as amended by the California Privacy Rights Act (collectively, the “CCPA”). It explains the categories of personal information we may collect through our website, intake forms, lead forms, telehealth-related forms, communication tools, SMS/text programs, email communications, and related digital services; the purposes for which that information is used; whether it is sold or shared; and how long it is retained.
Information that constitutes protected health information governed by HIPAA, or medical information governed by the California Confidentiality of Medical Information Act, is exempt from the CCPA and is instead handled in accordance with the euVita Medical Group PLLC Notice of Privacy Practices and applicable law.
1. Categories of Personal Information Collected; Retention
The table below lists the categories of personal information we may collect, whether we sell or share each category, and the criteria used to determine the retention period for each category.
euVita does not sell personal information. Where the table below indicates information is shared, that sharing is limited to euVita’s own marketing and advertising (such as retargeting) and can be controlled through the cookie preferences on our website.

2. Purposes for Collection and Use
euVita may collect and use personal information for the following business and commercial purposes:
- To operate, maintain, and improve the website and digital services.
- To process intake forms, lead forms, service requests, appointment requests, and user inquiries.
- To provide, arrange, support, or coordinate telehealth-related services and administrative services.
- To communicate with users, patients, prospective patients, and authorized representatives.
- To send administrative, transactional, service-related, healthcare-related, or legally required communications.
- To send marketing emails, SMS/text messages, or promotional communications where permitted by law and, where required, with consent.
- To maintain consent records, opt-out records, unsubscribe records, suppression records, and compliance documentation.
- To comply with legal, regulatory, contractual, professional, and healthcare-related obligations.
- To protect the rights, safety, security, systems, and business operations of euVita, its users, patients, personnel, and service providers.
- To improve website functionality, user experience, marketing effectiveness, and communication workflows.
3. Sensitive Personal Information
Some information collected through intake forms or telehealth-related forms may be considered sensitive personal information under California law. euVita collects and uses sensitive personal information only for the purposes permitted by Section 7027(m) of the CCPA regulations, such as providing requested services, processing intake submissions, supporting telehealth-related services, security and integrity, quality and safety, legal compliance, and recordkeeping, and does not use or disclose sensitive personal information for purposes that would trigger the right to limit under the CCPA.
4. Sale or Sharing of Personal Information
euVita does not sell your personal information. euVita uses certain limited information, primarily online activity information, for its own marketing and advertising, including retargeting through providers such as Meta. Under the CCPA, this use may be considered “sharing” for cross-context behavioral advertising. You can control or withdraw it at any time through the cookie preferences and consent tools on our website. euVita does not provide health condition details to advertising providers, and does not sell or share the personal information of consumers it knows to be under sixteen (16) years of age.
5. California Privacy Rights
Subject to applicable law and exceptions, California residents have the right to know, access, correct, and delete personal information; the right to limit certain uses of sensitive personal information; the right to opt out of the sale or sharing of personal information; and the right not to receive discriminatory treatment for exercising these rights.
